Certificate of Attendance

Upon completion of any SOCOG training, each attendee will receive a certificate of attendance. Please file your certificate(s) in a safe place. A fee of $5 per certificate will be required to get a replacement. To request a replacement, complete the Request for Certificate form and send payment of $5 per certificate made out to SOCOG.
